Cosmo Barnado, Monongah No. 3 Mine, was shoveling coal when slate from roof fell on him. He had timbers on hand and was instructed to use them, but failed to keep the place properly timbered. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Clara, John (Polish; 3 yrs exper; age 38; Married) September 5, 1905; died October 7, 1905. John Clara, Middleton Mine, was loading car under a piece of draw slate which let go, knocking out a post and striking him on the back. The slate men were sent in a short time before to take down the slate, but Clara said he would post it until he loaded his coal out. He posted the place, but not sufficiently to prevent one corner from letting go. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Chachura, Dom (Lithanian; 4 months exper; age 20; Single) September 12, 1905; died December 29, 1905. Dom Chachura, Monon- gah No. 6 Mine, was bringing loaded car up side track to drop it in on loaded track. In some way he was caught between the car and rib and was rolled about one car length. Should have been on the left side of car coming out instead of right side. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Windos, Joe (Italian; 13 days exper; age 26; Single) November 18, 1905; died November 28, 1905. Joe Windos, Montana Mine, was working by himself. He had lit the squib and called out fire, but the shot did not go and it is supposed he lit the same squib again or that the shot went off about the time he got up to it, which blew him 17 feet from the face and knocked out five props. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Swing, Charles (American; 2-1/2 yrs exper; age 28; Single) December 14, 1905; died December 18, 1905. Charles Swing, Shaft Mine, a driver, struck horse with his cap and horse kicked him in the stomach. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Beafore, Dominick (Italian; 3 yrs exper; age 15; Single) December 27, 1905. Dominick Beafore, New England Mine, left his place of work at 12:00 o'clock and rode to outside of mine to get his dinner. Upon returning to his work he got on front end of mine loco- motive and upon reaching a cross-over of main heading he jumped from the motor, of his own accord, evidently intending to throw switch. In so doing he in some manner fell on the track and was run over by the motor. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Madai, Mike (Hungarian; 1 yr exper; age 24; Single) January 2, 1906. Mike Madai, Montana Mine, had the cross cut in about 7 feet and it was between 10 and 11 feet wide. He had loaded his car and sat down when a piece of slate about 5 feet square and 12 inches thick fell on him. The mine boss had told him to set a prop and he said he would, but failed to do so. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Burdess, Tom (English; 1 yr exper; age 27; Single) January 30, 1906. Tom Burdess, Pennois Mine, a driver, was going in the room with empty car, the car struck a post that was supporting a cross timber, knocking it out and letting down about one-quarter ton of slate, which fell on him. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Alexander, James (Italian; 18 months exper; age 22; Single) February 15, 1906. James Alexander, Monongah No. 6 Mine, after tamping powder, took his lamp from cap and lit squib and failed to get away from the place before the powder exploded. Had the squib been lit at the end instead of back from the end the man would have had suf- ficient time to have gotten into cross cut. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Chiska, Andy (Hungarian; 5 years exper; age 37; Married) February 26, 1906. Andy Chiska, Montana Mine, was shearing some coal and the slate man came into his place and pulled some loose slate down and also wanted to pull this piece down, which afterwards fell. It weighed about 4 or 5 tons. Chiska would not let him; said the slate was not loose. The slate man told him to set a prop under it, as it was danger- ous, but Chiska said he was not afraid of its falling and went back to digging, and while the slate man was still arguing with him about setting a post the slate fell, catching Chiska. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Caustic, John (Polish; 10 yrs exper; age 35; Married; 3 children) April 9, 1906. John Caustic, Middleton Mine, went to work as usual in the morning. When within 10 feet of the end of the pillar where he worked, sat down to change his shoes, and while thus engaged about 100 pounds of draw slate and top coal fell, killing him instantly. Place was in very good condition. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Goff, Oscar (American; 3 wks exper; age 22; Single; $100.00 Insurance) May 9, 1906. Oscar Goff, Stafford Shaft, a driver, was coming out of heading with loaded trip of two cars. Seems that he was riding between cars and when reaching No. 1 face heading he got off to open trap door, got off on wrong side, and when running ahead was caught between car and rib. - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - - Lockhart, V. O. (American; 1 yr exper; age 25; Married; 1 child) June 15, 1906. V. O. Lockhart, Anderson Mine, was operating a small friction drum that is located under empty track of tipple, just outside the tipple house. This drum is used to lower empties and hoist timber and ashes from timber yard to mine track, at top of hill, for use in the mine. On this occasion ah empty car was ready to be lowered to timber yard. Lockhart took hold of friction lever and told them to push the car onto the incline.
